If your relative lives within the geographic area in which we provide services, we will provide a free assessment. We follow confidentiality guidelines as established by HIPPA. We will go to a residence or facility, whichever is more convenient.

Once the family hires Senior Solutions to provide services, we will send out a Letter of Agreement restating the basics of what we believe has been discussed. We sign the document, you don't. It's serves as a frame of reference for you, outlining the level of services we agreed to provide. It clearly states that our services can be canceled with a phone call.

Our standard Letter of Agreement contains no trick clauses, and has no legalese to decipher. It's written in plain old everyday American English.

Why wait?

It will be extremely helpful to have the following information before you call us:
 
Create a clear picture in your mind of exactly what you wish to accomplish. Decide on the best outcomes you can expect, the most important goals and what you envision the role of the Geriatric Care Manager to be.
 
Try and find out your relative’s health insurance and/or long term care insurance information, and their Medicare #. (Please note that many seniors believe they are on Medicare, even when they’ve enrolled themselves in an HMO).
 
Try and find out doctor’s names and phone numbers. A list of their medications would also be very helpful. (Please note that seniors will feel threatened by the perceived loss of their independence, and may not be forthcoming; tread lightly).
 
If you are calling us there has been a crisis of some sort. Who’s been helping your relative to this point? Are there local friends or other relatives involved in their care?
 
Although every client is different, the process begins with an evaluation. The social worker who will actually provide the assessment will call you to discuss the process BEFOREHAND. So, please be ready to let us know when it’s a good time to call and what phone number is best.
